What benefits does Blue Cross offer?

Do I have a claiming deadline?

For most plans, a specific period of time for allowable expenses to be submitted is indicated. Check the benefits booklet to find out the length of time you have to submit your expenses.

How do I file a claim?

You can obtain claim forms for Life, AD&D or Critical Illness benefits through your group administrator. Alternatively, you can contact Medavie Blue Cross at 1-800-667-4511 to obtain the claim form.

After completing the claim form, you should submit your claims to Medavie Blue Cross through your group administrator or by mailing, faxing or scanning it to the address indicated in the claim form.

What happens to my coverage if I retire or leave my company?

Coverage for critical illnesses will be terminated at age 70 or when your employment terminates, whichever comes first.

Coverage for life will be terminated when you retire or when your employment terminates, but if you are on or below 65, you and/or your spouse can still purchase an individual life policy from Medavie Blue Cross.

Coverage for AD&D will be terminated when your employment terminates or at the age of 65, whichever comes first.

How do I apply for a change in coverage amount?

You can make a change in your coverage amount by completing an Optional Benefits enrolment form and submitting it online. You can also submit through your group administrator.
